Therefore, Braid defined hypnotism as a state of mental concentration that often leads to a form of progressive relaxation, termed "nervous sleep". Later, in his The Physiology of Fascination (1855), Braid conceded that his original terminology was misleading, and argued that the term "hypnotism" or "nervous sleep" should be reserved for the minority (10%) of subjects who exhibit amnesia, substituting the term "monoideism", meaning concentration upon a single idea, as a description for the more alert state experienced by the others.

In conventional hypnosis, you approach the suggestions of the hypnotist, or your own ideas, as if they were reality. If the hypnotist suggests that your tongue has swollen up to twice its size, you'll feel a sensation in your mouth and you may have trouble talking. If the hypnotist suggests that you are drinking a chocolate milkshake, you'll taste the milkshake and feel it cooling your mouth and throat. If the hypnotist suggests that you are afraid, you may feel panicky or start to sweat. But the entire time, you are aware that it's all imaginary. Essentially, you're "playing pretend" on an intense level, as kids do.
One well-known example of a relaxation technique is known variously as progressive muscle relaxation, systematic muscle relaxation, and Jacobson relaxation. The patient sits comfortably in a quiet room. He or she then tenses a group of muscles, such as those in the right arm, holds the contraction for 15 seconds, then releases it while breathing out. After a short rest, this sequence is repeated with another set of muscles. In a systematic fashion, major muscle groups are contracted, then allowed to relax. Gradually, different sets of muscle are combined. Patients are encouraged to notice the differences between tension and relaxation.

In the brain, this state looks stranger still. A landmark study in the prestigious journal Science in the late 1990s, led by Pierre Rainville of the University of Montreal, described a study in which hypnotized people briefly placed their left hand in either painfully hot water, heated to 116 degrees Fahrenheit, or room-temperature water. Some of them had been told that they would be experiencing pain, but that they wouldn’t be very bothered by it — if, on a scale of one to ten, the hurt would normally register at an eight, they’d feel it as if it were a four. As all the participants placed their hands in the 116-degree water, their brains were scanned. The results were clear: Those who had been told that the pain would be less intense showed less activity in their brains — specifically, in the anterior cingulate cortex, which is associated with pain processing.
We also wonder how the suppression mechanism in PHA relates to the vast array of forgetting in the laboratory and in the world? Whereas some forgetting is seen as strategic, effortful and conscious (say, suppression), other forgetting is seen as automatic, effortless and unconscious (say, repression). Having mapped the common features of PHA and functional amnesia, we now need to explore and compare in greater detail their common processes (such as strategy use, motivation, level of awareness).
We have, however, come a long way from the days of Mesmer’s animal magnetism. The increasing interest in mindfulness meditation suggests that mainstream acceptance of the mind-body connection is growing. This year, two well-received books by serious science journalists, Marchant’s Cure, out in January, and Erik Vance’s Suggestible You, out this month, explore this territory — the demonstrable results of hypnosis, faith, and even magic — long dismissed as pseudoscience or explained away as the placebo effect. Just last month, NPR reported that placebo pills work even when people know they’re taking a placebo. “Those are real, biological changes underlying those differences in your symptoms,” Marchant told Science of Us earlier this year. It may here be requisite for me to explain, that by the term Hypnotism, or Nervous Sleep, which frequently occurs in the following pages, I mean a peculiar condition of the nervous system, into which it may be thrown by artificial contrivance, and which differs, in several respects, from common sleep or the waking condition. I do not allege that this condition is induced through the transmission of a magnetic or occult influence from my body into that of my patients; nor do I profess, by my processes, to produce the higher [i.e., supernatural] phenomena of the Mesmerists. My pretensions are of a much more humble character, and are all consistent with generally admitted principles in physiological and psychological science. Hypnotism might therefore not inaptly be designated, Rational Mesmerism, in contra-distinction to the Transcendental Mesmerism of the Mesmerists.

To just let it happen,” Patterson said. “And when you do this, the amazing thing is that it’s as if you’re talking directly to the part of the brain that’s monitoring the reactions.” In his work, he ties suggestions of comfort to the daily practice of caring for burn wounds. “In burn care you know they’re going to pull off the bandages and then they’re going to start washing the wounds,” he explains. “The message is that when your wounds are washed, that will be the reminder of how comfortable you are.” The patient will often look like they’re asleep. “But if you ask them, ‘If you can still hear me, feel your head nod,’ almost always you’ll get that head nod,” he said. He’s seen this work for decades, but is so grateful for the recent advent of brain-imaging studies. They serve as evidence he can hold up to skeptics: See? Do you believe me now?
At the outset of cognitive behavioural therapy during the 1950s, hypnosis was used by early behaviour therapists such as Joseph Wolpe and also by early cognitive therapists such as Albert Ellis. Barber, Spanos, and Chaves introduced the term "cognitive-behavioural" to describe their "nonstate" theory of hypnosis in Hypnosis, imagination, and human potentialities. However, Clark L. Hull had introduced a behavioural psychology as far back as 1933, which in turn was preceded by Ivan Pavlov. Indeed, the earliest theories and practices of hypnotism, even those of Braid, resemble the cognitive-behavioural orientation in some respects.

Preliminary research has expressed brief hypnosis interventions as possibly being a useful tool for managing painful HIV-DSP because of its history of usefulness in pain management, its long-term effectiveness of brief interventions, the ability to teach self-hypnosis to patients, the cost-effectiveness of the intervention, and the advantage of using such an intervention as opposed to the use of pharmaceutical drugs.
Experiments by researcher Ernest Hilgard demonstrated how hypnosis can be used to dramatically alter perceptions. After instructing a hypnotized individual not to feel pain in his or her arm, the participant's arm was then placed in ice water. While non-hypnotized individuals had to remove their arm from the water after a few seconds due to the pain, the hypnotized individuals were able to leave their arms in the icy water for several minutes without experiencing pain.
Finally, the neural underpinnings of PHA will be even clearer when we incorporate its most important aspect in imaging studies—the dissociation between implicit and explicit memory. In PHA (and in functional amnesia) the person is unable to explicitly recall certain information, yet we see evidence of this material on implicit measures. For instance, a participant given PHA may fail to recall the word “doctor,” learned earlier, but will have no trouble completing the word fragment “d _ _ t _ r”. Mendelsohn et al. did not assess implicit memory. Rather, they tested recognition, which in a sense confounds explicit and implicit memory. We’d like to compare brain scans of a PHA group trying to explicitly recall the movie (they should show reduced activation, as above) with brain scans of the same group completing an implicit memory measure of the movie (they should show normal activation). This would be tricky to do—implicit measures of complex material such as movies and autobiographical memories are hard to find or construct. But it would contribute to a more complete neural picture of the processes involved in these fascinating forms of forgetting.
